The brow and the forehead are often the first region to show signs of aging. Heaviness and drooping of the eyebrows can give us a tired and sometimes angry look. Drooping of the eyebrows also produce heaviness of the upper eyelids and hooding of the upperlid skin if patients have an existing supratarsal fold.
As the eyebrows sag and cause hooding of the upper eyelid, patients unconsciously elevate the eyebrow which causes both dynamic and static horizontal lines to appear on the forehead. Repeated frowning can also cause deep vertical creases to form between the eyebrows. Sagging of the skin in the corners of the brow increases the appearance of the crow’s feet.
By repositioning the brow to a more natural position, the eyes will appear larger and less tired. The position of the brow is different in men and women so it is important to have a skilled plastic surgeon evaluate each patient and determine the necessary position of the brow. Endoscopic Brow lift:
Endoscopic Brow lift is a technique perfected at Wave Plastic Surgery by Dr. Peter Lee. Most patients are candidates for this technique. Several small incisions inside the hairline are made. Then using a small telescopic camera and special instruments, the entire forehead is mobilized while visualizing it on the monitor. This frees up the forehead, so that we may move the brow up without much tension. Also in conjunction, the corrugator muscles are transected to smoothen the vertical lines between the eyebrows. Then to keep the brow in its new position, a special device called ENDOTYNE is used to fixes the forehead tissue to the bone. The device disappears after several months so that there is no permanent foreign body. The new position is kept with reattachment of the entire undermined surface. The advantages of endoscopic brow lift are minimial scarring, minimal discomfort and quick recovery time.
Open Brow lift:
For patients with excessive brow ptosis with excess skin, the open brow lift technique is preferable This surgery is accomplished by making an incision across the top of the head behind the headline . The incision is camouflaged into the hair, and no hair is shaved. Just as the endoscopic approach, the forehead soft tissue is elevated but under direct vision and for people with frown line, the corrugator muscle is transected as well. The forehead is then elevated, and excess skin is excised if necessary. Using this technique, the forehead can be lengthened or shortened. Again, the final strength of the repositioned forehead is achieved by the attachment of the undermined surface to the new position using ENDOTYNE device. The advantage of this procedure is that maximum benefit can be obtained for patients with severe ptosis of forehead and excess skin.
Step 1 - Anesthesia
Medications are administered for your comfort during the surgical procedure. The choices include intravenous sedation and general anesthesia. Your doctor will recommend the best choice for you.
Step 2 - The incision
A brow lift may sometimes be performed using an endoscope (surgical video device) and special instruments placed through small incisions made within the hairline. This allows the tissue and muscle beneath the skin to be repositioned, altered or removed, correcting the source of visible creases and furrows in the forehead.
Correction of a low-positioned or sagging brow may be made with or without the use of an endoscope through incisions at the temples and in the scalp.
This technique may be done in conjunction with incisions hidden within the natural crease of the upper eyelids to eliminate frown lines between the brows, on or above the bridge of the nose.
An alternative brow lift technique is the coronal brow lift. The coronal brow lift can pinpoint specific regions of the brow to correct.
This technique involves an incision from ear to ear, lifting the forehead and removing excess skin from the scalp.
Recovery time is often longer than the endoscopic brow lift due to the size of the incision.
The incision lines from a brow lift are well concealed within the hair or natural contours of the face unless they are placed at the hairline to shorten the forehead.
Results appear gradually as swelling and bruising subside to reveal smoother forehead skin and a more youthful, restful appearance.
